New Client Document Request

We need to gather several documents to help us prepare your financial plan.   We know that gathering these documents requires effort on your part, and we greatly appreciate it.


The Documents We Need

Please note that this is a standard list, and that some may not apply to your situation.  Similarly, if we need additional documents, we will request those separately.

  • Last two years’ tax returns
  • Last two pay stubs
  • Recent retirement (ie 401k, IRA, Roth, SEP, Simple) plan statements
  • Recent taxable brokerage statements
  • Recent annual life insurance statements
  • Recent annual long term care insurance statements
  • Recent pension estimates (or statements)
  • Recent Social Security statements or online estimates
  • Employer benefit information (retirement plans, group insurance, etc.)
  • Recent mortgage/HELOC statements
  • Current estate planning documents



Getting Your Documents to Us

There are several different ways to get your documents to us!

For electronic documents, please upload securely through our website.

For hard copy documents, you can send us a FAX at 888-731-7290, or mail them to:
